1. Download the document below. You should either see a screen pop up indicating it has been downloaded or you may see the document downloading on the bar at the bottom of your computer where it will allow you to select "Open" or "Show in Folder".
Then either:
- Enter the information on your computer, save the document to your computer after the information has been inserted, and attach the document to an email addressed to Shelley Murphy shelleym@mtaflcio.org
- Print the doument. Fill the information in by hand. Fax it to 406-449-3324 or scan it and email it to Shelley Murphy shelleym@mtaflcio.org
- If you already maintain a document that has all of the requested information. Don't bother filling out our form. Just fax or email us the document you have that lists your officers and staff with contact information and officer/job titles, etc.
